Wellness Monitoring Program - South Island Health Area
Condition Specific Support, Home Health Care, Public Health, and Seniors Services
Provided by Island Health
Wellness Mentors are Occupational Therapists and Recreation Therapists who use a holistic approach to improving health, which includes physical, emotional and spiritual wellness. They are able to help establish accountability within the participant, as they work towards their individual goals.
What to Expect
- Wellness Monitors communicate with Primary Care Providers as needed, to ensure seamless care.
- Connection and referrals to appropriate community programs and resources
- Wellness Mentors take a client-centered approach to discussing and exploring the concerns of the participant
- Support and guidance is provided to create achievable goals and action plans, that can be regularly followed-up on
- Guidance is provided to address barriers to achieving goals and plans, such as social isolation, physical limitations, transportation and finances
- Wellness Monitors will make regular phone calls to check in on the the participants progress, and to discuss any health changes and/or concerns
- Participants must be at least 19 years old
- The Wellness Monitoring Program is currently only available for those living in the south of Vancouver Island
- Individuals needing support to manage their health and wellness for reasons such as: Lack of family/friends and feeling alone; memory loss; chronic pain; wanting to make life changes but needing support to do so
- Interested individuals should speak with their Primary Care Provider
- Physicians or Nurse Practitioners may call the Practitioner or Community Connectors (telephone numbers below) to place a referral
