Occupational and Physical Therapy - Community Health Services (CHS)

Physiotherapists and Occupational Therapists work with the patient to restore, improve, or maintain physical independence and safety.
Community Rehabilitation Services are provided by physiotherapists and occupational therapists in the home and community. Community therapists work with those with complex physical, mobility, cognitive and/or environmental needs to optimize physical recovery and activities of daily living. The need for hospital visits is reduced with the assistance of physio and occupational therapists, as they provide physical rehabilitation, proactive care and associated supports.

Services Include:
  • Improve muscle strength, balance, and coordination,
  • Develop approaches to self-manage pain,
  • Maintain independence at home for activities of daily living and self-care,
  • Consider strategies to make the home more accessible and safe,
  • Can offer mobility assessments, prescribe and support the use of equipment and adaptations in the home,
  • Community therapists can offer other clinical assessments, including falls screening and assessment, and recommend falls risk reduction interventions.

Eligibility & Criteria
  • Patient must be a resident of British Columbia
  • Assessment must occur in the home environment
  • Patient has a physical disability/limitation
  • Patient must be a Canadian citizen, or have permanent resident status
  • Patient is recovering from surgery/hospital visit and requires care following discharge
  • Patient needs rehabilitation or therapy to support their independence & safety at home
  • Patient requires care at home rather than hospitalization, or care because of a terminal illness

How to Access
  • No referral is needed, though a primary healthcare provider can refer their patient to the program (see provider information below)
  • For patients with a Home Health and Community Health Services Case Manager, please contact them directly as they will provide local information tailored to any specific needs
  • Please locate the appropriate local region below, and call the associated Community Access Line to learn how CHS Case Managers can provide support for the patient, family of a patient, or for providers looking to refer their patient to access CHS
