Outpatient Supports - Campbell River Hospital Wellness Centre

Provides a variety of outpatient and chronic disease management services and classes out of the Campbell River North Island Hospital.
The Campbell River Hospital Wellness Centre provides a variety of outpatient and chronic disease management services and classes based on the needs of each individual. While located in the Campbell River North Island Hospital, programs operate on an outpatient, triage-based system with varying wait times. The multidisciplinary team is available to provide the necessary services, including for those who are hospital involved, or are likely to be hospital-invovled.

Programs vary depending on the needs of the client, but may include:
  • Indigenous health and community nutritionist and liaison nurses
  • Clinics such as the Bone & Joint Clinic, Maternity Clinic, Kidney Care Clinic and Stroke Clinic
  • Dietitian support for Outpatient Nutrition Services. Referral required from primary healthcare provider (FP/NP) or other clinician (providers see below)
  • Condition-specific programs include the Cancer Care Program, Diabetes Education, Medical Day Care, Telehealth, and Positive Wellness HIV & North Island Liver Services
  • Heart Health Education: Individual support provided by a registered nurse (RN), and access to the Take Heart Breathe Well program. Referral required from primary healthcare provider (FP/NP) or other clinician (providers see below)
  • Chronic Disease Management: Individual management and support by a registered nurse (RN) for chronic conditions, chronic pain, and access to the CDM exercise program. Referral required from primary healthcare provider (FP/NP) or other clinician (providers see below)
  • Social Work: Includes outpatient psychosocial supports such as counselling, disability applications (PWD, DTC, CPP-D), advocacy, financial assistance (taxes, funding, Pharmacare), referrals, and other health-related resources. Referral required from primary healthcare provider (FP/NP) or other clinician (providers see below)
